I’m a first time patient what do I need?

Bring a photo ID, and if you have an insurance card bring that along too. If you have been to a dentist recently and had x-rays taken make sure to bring a copy with you, or e-mail them to advanced.dental.cosmetics@gmail.com before your appointment. The rest of the forms you will fill out at our office before you are seen by the dentist.

Is your office Accessible?

Bayside Office: Yes, we are handicap accessible and have a ramp system.

Manhattan: Yes. Elevator available.

What should I do if I require premedication?

Please be sure to request a prescription prior to your appointment, or if you are unsure, contact us and we can help.

Does Your Office Offer Financing?

Please contact us to discuss the options we have available.

What payment methods does your office accept?

Cash, personal check, money order and debit/credit card (MasterCard or Visa only). Patients can also call us to pay bills over the phone with a debit/credit card.

Do you provide Parking?

Bayside Office: Yes,
We have a parkway right outside our office, which is reserved specifically for our patients!

Note: Please do not park in the shopping center parking across the street, for your car could be towed.

Manhattan Office: No

Do you OFFER Invisalign® Services?

Yes we do! In our Bayside office, Dr.Huh is trained in Invisalign® and has completed many successful cases. Come in, or schedule your consult to see if Invisalign® is an option for you.

How long will my first appointment last?

Just about 45 Minutes.
